Trouble Report

Please also note the following:
If you're reporting a malfunction without reporting any loss (loss of balance or cash), please only fill in Part 1.
Otherwise, please also fill in Part 2.

Part 1: Machine Details
Number which is stuck onto the machine. If you can't find the number, please describe the machine.
Name of canteen or café, name of building or name of street and city.
Details of what happened, apparent defects, screen text, acoustic or visual notifications/irregularities.
In the case of trouble topping up your card, holding the card up to the card reader again can prompt a second topping-up process. Did you try that?
